How to Report Damaged Books

When to report a damaged book

The Damaged book report form is intended for CELA patrons to report issues with books in any format in our collection, including audio, embossed braille, electronic braille, and text. By providing details about the problem, you help us ensure our materials remain accurate, accessible, and enjoyable for all readers. Please complete the form with as much information as possible, including any specific details about the damage or error. This will help our team investigate and resolve the issue more quickly.

Instructions for completing the Damaged book report form

From a search:  

  • Search for the title you wish to report. 
  • From the Search results page, select the title you wish to report to go to the title's details page with more information about the title. 
    • Note: CELA may offer multiple versions and/or formats of the same title. Please make sure you have selected the correct title and format you want to report. 
  • On the selected Title details page, move to the bottom of the page and select the link to “Report a problem with this book”. 
  • The link will bring up a Damaged book report that has the title, author and book number fields already populated. Your account number will also be populated if you are logged in. Please add the missing details about the problem you encountered. 

From your History page: 

  • Log into your CELA account from the CELA home page. 
  • Go to the History page under My account. This page may take a few minutes to load.
  • Select the title you wish to report. You will go to the Title details page for that book.
  • Move to the bottom of the Title details page and select the link to “Report a problem with this book”.  
  • The link will bring up a Damaged book report that has the title, author, book number, and your account number fields already populated. The remaining information will need to be filled out.
