By Mary Potter Kenyon. 2014
When Mary Potter Kenyon’s husband David was diagnosed with cancer in the summer of 2006 she searched libraries and bookstores…for books on cancer and the caregiving experience. What she discovered was a plethora of technical and medically-oriented books or those written by a caregiver whose loved one had died, a scenario she refused to contemplate. While serving as David’s companion during Wednesday chemotherapy treatments, Mary began journaling about their experience as a couple and parents of young children as they navigated the labyrinth of cancer. It soon dawned on her that she was writing the very book she had searched for upon David’s diagnosis: one that goes beyond the cancer experience to give hope and inspiration to the reader. Chemo-Therapist: How Cancer Cured a Marriage is a moving testimonial of a relationship renewed by the shared experience of a life threatening illness." Initially, after David’s diagnosis, I would cringe when I read books or articles by cancer survivors who stated that cancer had been a gift in their lives. How could all that David endured be viewed as a gift? The invasive surgery, the weeks of chemotherapy and radiation: a gift? Yet, after the cancer, David would often reach for my hand and say, “If it is cancer that is responsible for our new relationship, then it was all worth it.” And I’d reluctantly agree that cancer had been a gift in our lives. We’d both seen the other alternative: patients and survivors who had become bitter and angry, and neither one of us wanted to become that.

Cleft lip and palate represents the most common congenital facial anomaly. Congenital anomalies are one of the top 10 causes…of disability and suffering in many low and middle income countries (LMICs.) Further, an estimated 30% of the global burden of disease is treated primarily through surgery, and among surgical conditions, treating clefts conveys disproportionately large benefit relative to cost because suffering can be halted at a young age and deliver a lifetime of health. Over the last 50 years, cleft care has advanced in low-resource settings through international surgical missions and the efforts of sparse local providers. While exciting anecdotes of impact abound from these efforts, recent studies have identified a complication rate that often exceeds 50% and there is concern about the sustainability of foreign volunteer missions. This book serves as a thorough resource for providing medical and surgical care of cleft lip and palate deformities in low-resource settings, geared to the context of those settings. The content is consensus-based and evidence-based, with emphasis on treatment that is appropriate to a low-resource context and how this differs from that in higher-resource environments. It is authored by recognized cleft experts in close partnership with providers who practice in low-resource settings. It is heavily pictorially based through illustrative figures, photos and diagrams, to be facile for those who speak English as a second language. Following the introduction, Complete Cleft Care in Low-Resource Settings has six main sections: patient workup, surgical treatment, comprehensive care, educational innovation, care delivery models, and finally a series of illustrative case studies. Uniquely, the book’s breadth encompasses cleft care comprehensively, but does so through a practical “how to” format with many “from the field” examples of what has worked.

This book is the first to introduce applied behavioral, social, and health sciences researchers to a new analytic method, the…time-varying effect model (TVEM). It details how TVEM may be used to advance research on developmental and dynamic processes by examining how associations between variables change across time. The book describes how TVEM is a direct and intuitive extension of standard linear regression; whereas standard linear regression coefficients are static estimates that do not change with time, TVEM coefficients are allowed to change as continuous functions of real time, including developmental age, historical time, time of day, days since an event, and so forth. The book introduces readers to new research questions that can be addressed by applying TVEM in their research. Readers gain the practical skills necessary for specifying a wide variety of time-varying effect models, including those with continuous, binary, and count outcomes. The book presents technical details of TVEM estimation and three novel empirical studies focused on developmental questions using TVEM to estimate age-varying effects, historical shifts in behavior and attitudes, and real-time changes across days relative to an event. The volume provides a walkthrough of the process for conducting each of these studies, presenting decisions that were made, and offering sufficient detail so that readers may embark on similar studies in their own research. The book concludes with comments about additional uses of TVEM in applied research as well as software considerations and future directions. Throughout the book, proper interpretation of the output provided by TVEM is emphasized.Time-Varying Effect Modeling for the Behavioral, Social, and Health Sciences is an essential resource for researchers, clinicians/practitioners as well as graduate students in developmental psychology, public health, statistics and methodology for the social, behavioral, developmental, and public health sciences.
